How much do appointments cost?
We spend extensive time with you on your wellness plan. Insurance companies do not value this time, which is why we do not bill them for our services. We provide you with an invoice for you to submit to the out of network portion of your insurance for possible reimbursement. See the insurance question below for more details.
Do you accept my insurance?
As a Specialized Functional Medicine practice we do not accept insurance or bill insurance companies. After each appointment, you’ll be given an itemized health care receipt which you can submit to your insurance for possible reimbursement. We recommend calling your insurance company prior to your visit to find out what they will cover. Tell them the following:

What are my payment options?
We accept all credit cards, debit cards, HSA/FSA and Care Credit, cash, and check. Note: If you have a phone or virtual telehealth appointment, we require a card on file to process payment directly after your appointment. You can also log into the portal right after your appointment and make a payment directly. We appreciate your prompt payment!
Is there a charge to cancel or reschedule my appointment?
If you need to cancel or reschedule your follow up appointment, please allow us 24 hours notice to avoid a $50 cancellation fee. Call or text 801-797-0877 to cancel or reschedule.
How should I prepare for my first visit?
We like to be prepared for your first visit by reviewing all of your intake forms, concerns, past history and prior testing ahead of time. We kindly ask you to complete the intake forms in the Patient Portal at least 24 hours before your appointment to maximize your time with your provider. See instructions on the Questionnaire tab in the portal for filling out the forms. We recommend completing the intake forms on a computer instead of your phone so you can see the whole form with ease. Please remember to save often when completing the forms.

What is the cost of lab work?
Investigation of your inflammation and health stressors is an essential part of getting to the root cause of your concerns. You will speak with your provider and decide what set of labs will provide the best insight to your health situation. Most functional medicine labs take a deep dive into your health are not covered by insurance. Lab costs vary, depending on what labs you need to investigate your specific health needs. We choose the highest quality labs and provide you with cost-effective self-pay options. Our office does not bill insurance for lab work.

If you have insurance coverage for labs, there might be times when we will send you to LabCorp and they can bill your insurance. To understand whether your insurance company will cover lab work, we recommend calling your carrier directly. They can help you determine your coverage, the amount of your deductible, whether your plan covers labs before or after your deductible is met, whether you have a co-insurance or a co-pay and what that exact amount is, and what lab is included in your plan. Knowing this information ahead of time can potentially save you time and money.

How can I get a copy of my invoice, lab results, or treatment plan?
You can always find your invoice, lab results and treatment plan in your Patient Portal. Our office provides hard copies of these forms at each visit and they can be emailed after virtual appointments, upon request. If you need us to send forms to other providers or create duplicates, the cost will depend on the time required as per our Convenience Fees.
Do you offer telehealth?
We are pleased to offer telehealth appointments to those who live in PA and UT, and we hope to add other states soon. Should you live outside of these two states, you can come in person to your first appointment to the UT office once a year and follow up can be over telehealth during the remainder of the year. Many people find telehealth to be a convenient way to receive the health and wellness care they need while remaining in the comfort of their own home. All supplements, lab work, prescriptions orders, etc. are submitted into the patient portal. Supplements can be shipped to the patient via Fullscript Supplement Dispensary available on our website. Blue Pebble Health utilizes an advanced medical record system which allows the patient to have access to their medical information through logging in to the patient portal.
What should I do if I have an emergency?
In case of an emergency, go to your local hospital emergency room or dial 911. As Functional Medicine Specialists we do not handle emergency care and we do not regularly check messages after hours or on weekends.
Do you help treat terminal or chronic illness?
There is no concern too big or too small. We are certain to uncover key pebbles of health knowledge to help you better navigate the journey wherever you are starting from.
